WHAT ARE THEY?
Cosmetic fillers are injectable substances used to fill in wrinkles and create volume. They were originally developed and used to fill wrinkles around the mouth and smile lines (nasolabial folds), but their use has expanded dramatically in the past 5 years. Fillers are frequently used in lips/lip lines, hollows under the eyes (tear troughs), cheeks, temples, chin, nose (to straighten/even out irregularities in the nasal bridge), hands and chest wrinkles. There are many fillers that are FDA approved in the U.S. and each of them has certain properties that make it the most effective in a particular area of the skin.
HOW DO THEY WORK?
Wrinkles are due to many things, including a loss of natural tissues in the underlying layers of the skin due to age, genetics, and environmental factors. An injectable filler is used to replace the loss of supporting tissue beneath the skin. The filler can be injected very superficially for fine lines or deeper for creating volume in an area. The skin is then lifted outward and the wrinkles in the area are reduced. Prior to injecting the filler, the skin is treated with a topical anesthetic cream and ice packs. The injections are then performed using very a fine needle or blunt-tipped cannula.
WHAT TO EXPECT AFTER TREATMENT:
The effect of most injectable fillers appears immediately and may last from 3 to 24 months, depending upon the product that is used and your body’s metabolism. The treatment area may be slightly red right after treatment, with some small visible red dots from the injections. About 10 to 20% of patients may also develop some small areas of bruising. Most patients feel comfortable returning to work or other daily routines directly from the office after the application of makeup. However, in some patients, lip augmentation with injectable fillers may create some significant swelling for 24 to 48 hours.

These are a group of injectables made of hyaluronic acid gels. Hyaluronic acid is a naturally occurring substance in your skin that restores volume to the face and diminishes the appearance of deep wrinkles and fine lines. This group of fillers can be placed in most areas of the face and results are seen immediately. There are slight differences in the properties of these fillers that give them specific value in filling different areas. Popular areas for use of these fillers includes nasolabial folds/smile lines, lips and lip lines, hollow tear troughs, cheeks, chin, jawline, and hands.
Sculptrais a biocompatible material that is injected into areas of volume loss including cheeks, temples and jowls for overall facial rejuvenation. Diluted Sculptra® can also be used to soften wrinkles in the décolletage. It gradually stimulates your own collagen growth, so the results are very natural, but not immediate. Several treatments are usually required to see the best results over time. It has one of the longest lasting results of all the fillers, usually not needing a touch up for 2-4 years. It is often combined with other fillers or laser therapy for complete rejuvenation.

Filler Injections with Microcannulae
The use of microcannulae to inject fillers has revolutionized our ability to volumize and lift the face with less risk of bruising, swelling, and damaging tissue. These instruments are the shape and size of a needle allowing our fillers to flow naturally, but they have a blunt tip to allow gliding through the tissue with less trauma. A needle is used initially to make a small opening in the skin through which the cannula can be inserted. Most fillers can be injected with this technique or it is used in combination with the usual small needle to get the most natural result.
